Everything you need to know about RVTS. A must-read document.

To claim for reimbursement of expenses, complete this form and send to the RVTS office along with supporting tax invoices. Expenses that do not include the supporting tax invoice will not be reimbursed.

Registrar intending to take leave from the program (not including annual leave), must complete a leave application form at least 8 weeks prior to the leave and send to the RVTS office for approval.

Any change in training location must be prospectively approved by RVTS. Complete the attached and fax it to the RVTS office.

This form allows access to Medicare. Complete a new form for each provider number, have it signed by your supervisor and fax to the RVTS office.

Complete this form if you are applying for a provider number for the first time, or you are renewing a 19AB exemption. The form and all supporting documents are to be sent directly to Medicare. A placement approval form must also be completed and faxed to the RVTS office after it is signed by the registrar and supervisor.

Use this form to apply for additional provider numbers. Send the comleted form directly to Medicare. You must also complete a placement approval form which is faxed to the RVTS office.

Membership of ACRRM is funded by RVTS. Fill out this form to also be enrolled in FACRRM.

Registrars may apply for recognition of prior learning towards FACRRM. This should be done in conjunction with their training advisor.

Once registrars have completed all training requirements for Fellowship of ACRRM they must complete this form and send it to the RVTS office along with all supporting documents

RVTS funds memebership of the RACGP. Send the completed application form to the RVTS office.

Registrars may apply for recognition of prior learning towards FRACGP. This should be done in conjunction with their training advisor.
